April 28, 2026 – Maine Parent Federation

Join Allison Wiest and Dylan Campbell from Maine Parent Federation to talk about Healthcare Transition Best Practices.

The transition from pediatric to adult healthcare is an important milestone for all young people. For youth and young adults with special healthcare needs, this shift also brings changes in how care is delivered, decisions are made, and responsibilities are shared. Planning ahead is key to supporting positive health outcomes, building independence, and ensuring continuity of care.

This training will help youth, families, and caregivers understand the healthcare transition process, including individual healthcare rights, what to expect when moving to an adult-centered model, and strategies for gradually shifting responsibility to the young person. Participants will leave with practical tools and knowledge to support a successful and confident transition into adult healthcare.
